SIGNATURE WORKSHOPS

Recognize Early: Changes in Condition

Subtle changes in condition are often the earliest indicators of decline—yet they are frequently missed, delayed, or attributed to baseline.

This workshop focuses on strengthening the clinician’s ability to detect early change, interpret its significance, and respond appropriately before it progresses into a more serious event.

Participants are guided through realistic scenarios where information is incomplete, requiring them to think critically, prioritize concerns, and determine next steps.

Participants will learn how to:

✔ Recognize early and often overlooked signs of decline
✔ Differentiate baseline from acute clinical change
✔ Interpret report and identify what requires attention
✔ Determine when escalation is necessary
✔ Communicate findings clearly and effectively using SBAR

Clinical Awareness. Early Recognition. Timely Response.

The Bulletproof Chart: Documentation & Clinical Communication

Documentation is more than a task—it is a clinical and legal tool.

Notes that are vague, incomplete, or disconnected from clinical reasoning can impact patient outcomes, communication, and regulatory exposure.

This workshop focuses on strengthening how staff document changes in condition and communicate clinical concerns in a way that is clear, defensible, and clinically meaningful.

Participants review real-world examples of documentation, identify gaps, and receive coaching on how to improve clarity and accuracy.

Participants will learn how to:

✔ Identify high-risk documentation patterns
✔ Write clear, clinically relevant, and defensible notes
✔ Connect observations to clinical reasoning
✔ Improve communication with providers through documentation
✔ Support continuity of care with accurate and timely reporting

Recognizing Change in Memory Care

Clinical Insight in Complex Presentations

In memory care populations, changes in condition are often expressed through behavior rather than clear clinical symptoms.

These changes are frequently misinterpreted, delayed, or dismissed as part of dementia—resulting in missed opportunities for early intervention.

This workshop focuses on helping staff recognize when behavior reflects an underlying clinical issue and how to respond appropriately.

Participants work through scenarios involving agitation, withdrawal, and functional change, learning how to assess, interpret, and escalate concerns.

Participants will learn how to:

✔ Identify changes from baseline in residents with cognitive impairment
✔ Recognize behavior as a potential indicator of illness or distress
✔ Differentiate behavioral symptoms from medical causes
✔ Document behavior clearly and clinically
✔ Communicate changes effectively to providers

Recognizing Infection Early

Clinical Awareness. Early Detection. Timely Response.

Infection in older adults often presents without classic signs such as fever or localized symptoms.

Instead, early indicators may include confusion, decreased appetite, weakness, or functional decline—symptoms that are easily overlooked or misattributed.

This workshop focuses on improving early detection of infection and strengthening the clinical response to subtle changes.

Participants work through case scenarios that require them to identify early warning signs, determine next steps, and communicate concerns clearly.

Participants will learn how to:

✔ Recognize atypical signs of infection in older adults
✔ Identify subtle clinical changes that indicate possible illness
✔ Differentiate normal aging from acute infection
✔ Determine when immediate intervention is needed
✔ Document and communicate findings effectively
